Guiding principle
The site is designed to collect the minimum personal information needed to book an appointment and answer enquiries. No detailed health information is requested by the site’s forms.
Information collected
Through the booking form: first and last name, phone, email, appointment address, service requested, preferred date and time window, whether an order is held, the client’s age for a pediatric appointment, and an optional practical note.
What the site does not ask for
The form does not ask for detailed medical information, health insurance card numbers, or photographs of medical documents. These are handled during the visit or through a validated secure channel.
Use
The information is used only to process the request, confirm the appointment, send preparation instructions and carry out the administrative follow-up of the service.
Retention and security
Forms must use appropriate security measures and the site is served over HTTPS. The booking platform selected must be suited to processing personal information. Retention periods will be specified once that solution is chosen.
Sharing
No information is sold. Sharing is limited to what the delivery of the service requires, notably with the partner laboratory where a specimen’s handling calls for it.
Your rights
You may request access to your personal information, its correction or its withdrawal, in line with the requirements applicable to Québec businesses. Contact details for the person responsible for the protection of personal information will be published before launch.
Analytics
Any audience-measurement tool will be enabled only with a configuration that respects the applicable privacy obligations.
